Do you have a copy of your child’s Health Plan?
The Health Plan takes forward any actions from the child’s annual health assessment and you should receive a copy. The Health Plan will outline any treatment the child is due to receive for existing health problems, and action required to prevent future problems.
If you haven’t received a copy please inform your Supervising Social Worker who will request this from the Local Authority.
